INSTRUCTIONS FOR SAMPLING &Â PROFILING WASTE MATERIALS ONLINE FORM
- Download the form below and complete one CSI Waste Profile Form for each waste stream.
- Attempt to answer all questions accurately and thoroughly.
- Pay special attention to the Process Generating Waste. Ex.: Are any chemicals used as solvents in your process? Is this a degreasing process? Is this material waste solvent or waste product? Or is this a paint related process?
- You may submit additional documents, inventory details lists or pictures by email to: tmcneilly@chemicalsolvents.com
- Please include a copy of the Safety Data Sheets for any component used in the process by which this waste material is generated. Any prior analytical data or a copy of a previous hazardous waste material manifest may also be provided; however, the inclusion of analytical results does not absolve CSI from the responsibility of analyzing the material.
